The Blazers trailed 64-49 early in the third quarter before getting back in the game with a 14-0 run led by Simons and Grant. Portland took a 71-67 lead following a three-point play from Grant with 5:45 remaining in the third. That bucket capped-off a 22-3 run by the Blazers.
Portland (7-19) outscored Phoenix 38-20 in the third quarter taking a six-point lead at 87-81 heading into the fourth quarter.
The Blazers would push their lead to 105-91 after a Malcolm Brogdon three-pointer with 4:26 remaining which finished an 11-4 run by the Blazers.
Phoenix (14-13) responded with a 13-2 run to cut the Blazers lead to three at 107-104 with 35 seconds remaining. Simons put the game away with a pull-up jumper with 10.1 seconds remaining.
Kevin Durant scored a game-high 40 points on 16-of-28 shooting and Devin Booker added 26 points and seven assists.
Deandre Ayton recorded a double-double with 16 points and 15 rebounds and Malcolm Brogdon scored 14 points on 5-of-10 shooting.
The Trail Blazers connected on 14-of-28 from beyond-the-arc while the Suns made just 5-of-23.
