Portland (9-18) took a 104-87 lead into the fourth quarter following a three-pointer from Simons at the buzzer, but Nikola Jokic and the Nuggets weren't fazed.
Denver (14-12) responded with a 16-2 run to cut Portland's lead to just three at 106-103 with 7:28 remaining. Later, the Nuggets used a 9-0 run to retake the lead at 122-120 with 59.8 seconds remaining.
Jokic scored with 16.8 seconds remaining to tie the game at 124. Chauncey Billups elected to use his final timeout to setup the final shot.
After the timeout, Simons dribbled the clock down to 4.7 seconds before making his move. He put the ball up and in off the window as time expired for the win.
Jokic led Denver with a game-high 34 points and Jamal Murray added 24 points nine assits.
Sharpe finished with 27 points and six rebounds and Deni Avdija recorded 19 points on 6-of-9 shooting.
